01Who this notice covers

This notice explains how Elite Endo ("Elite Endo", "we", "us") handles information about you. It applies to this website and to the ways you contact us through it, including the appointment request and referral forms.

Your medical and dental records are handled separately under federal health privacy law. Where the two overlap, the stricter protection applies.

02Information we collect

We only ask for what we actually need. Depending on how you use the site, that can include:

  • Contact details you enter yourself: name, phone number, email address, and preferred appointment times.
  • The reason for your visit, symptoms, or referral notes, where you choose to describe them in a form or a message.
  • Referral information submitted by a dental office on a patient's behalf, including the referring practice's own contact details.
  • Basic technical information your browser sends automatically, such as device type, browser, approximate region, and the pages you viewed.

We do not ask for payment card numbers, social security numbers, or insurance identifiers through this website. Please do not send them to us by email or through a web form.

03How we use it

Information you give us is used to respond to you and to provide care. Specifically:

  • To contact you about an appointment request, a referral, or a question you have sent.
  • To prepare for your visit and to provide, coordinate, and follow up on treatment.
  • To communicate with your referring or general dentist about your care.
  • To meet legal, professional, and record-keeping obligations that apply to a dental practice.
  • To keep the website working properly and reasonably secure.

We do not sell your personal information, and we do not share it with advertisers for their own marketing.

04Protected health information

Once you become a patient, information about your health, treatment, and payment for care is Protected Health Information under the Health Insurance Portability and Accountability Act, known as HIPAA.

That information is governed by our Notice of Privacy Practices, which is provided at your first visit and is available from the front desk on request. It describes in detail how protected health information may be used and disclosed, and the specific rights you have over it.

A website form is not a secure channel for detailed medical information. Please share only what is needed to book or triage, and save the rest for your appointment or a phone call.

05When we share information

We share information only where it is necessary or where the law requires it:

  • With your referring dentist or another treating provider, as part of coordinating your care.
  • With service providers who help us operate the practice and this website, such as hosting and communications providers, who are permitted to use the information only to perform that service for us.
  • With your dental benefit plan, where you ask us to help with a claim or an estimate.
  • Where we are required to by law, a court order, or a public health or regulatory obligation.

06Cookies and analytics

This site uses only what it needs to function and to understand general traffic patterns, such as which pages people find useful. That data is looked at in aggregate and is not used to build an advertising profile of you.

Most browsers let you refuse or delete cookies in their settings. The site will continue to work if you do.

07How long we keep information

Patient and dental records are kept for the period required by state law and professional standards, which is generally several years beyond the last date of treatment.

Enquiries that do not become appointments are kept only as long as they are useful for following up, and are then deleted.

08Security

We use reasonable administrative, physical, and technical safeguards to protect the information we hold, including encrypted connections to this website and restricted access to records.

No system is perfect, and no method of transmitting information over the internet is completely secure. We tell you that plainly rather than promise otherwise.

09Your rights

Depending on where you live and on whether the information is a health record, you may have the right to:

  • Ask what personal information we hold about you and request a copy of it.
  • Ask us to correct information that is inaccurate or incomplete.
  • Ask us to delete information, where we are not required to keep it.
  • Ask us to restrict certain uses or disclosures, or to communicate with you by a particular method.
  • Be free from discrimination or worse service for exercising any of these rights.

California residents have additional rights under the California Consumer Privacy Act as amended. Note that information already covered by HIPAA is handled under that law instead.

To make a request, contact the practice using the details below. We may need to verify who you are before we act on it.

10Children

We treat children as patients, and in that setting information is provided by a parent or guardian and handled as part of the dental record.

This website is not directed at children, and we do not knowingly collect information online from a child without a parent or guardian involved.

11Changes to this notice

If this notice changes, the updated version will be posted here with a new date at the top. Significant changes will be brought to patients' attention at the practice.

If you believe your privacy rights have been violated, you may raise it with us directly and you also have the right to complain to the relevant federal or state authority. We will not retaliate for a complaint.
